These Terms of Use (“Terms”) govern your access and use of the website and telehealth services provided by Premier Neuro Health. By using our site or services, you agree to these Terms.

1. Use of Website

This site provides information about neurological health and telemedicine services. It is intended for individuals residing in states where Dr. Souayah is authorized to practice as follows:

  • New Jersey (NJ) – Full medical license; direct patient care and telehealth authorized.
  • New York (NY) – Full medical license; direct patient care and telehealth authorized.
  • Massachusetts (MA) – Full medical license; direct patient care and telehealth authorized.
  • California (CA) – Full medical license; direct patient care and telehealth authorized.
  • Florida (FL) – Telehealth only under Florida Board of Medicine registration; no in-person care.
  • Illinois (IL) – Neurological second opinions only upon written referral; no direct management.
  • South Carolina (SC) – Neurological second opinions only upon written referral; no direct management.

International Patients

Premier Neuro Health provides cash-pay, advisory, and educational consultations to patients located outside the United States, including Europe, the Middle East, and other regions.
International services are permitted only where local law allows and are limited to second-opinion, educational, and wellness consultations.

No prescriptions, direct patient management, or diagnostic orders will be issued unless Dr. Souayah is duly authorized in the patient’s country or the consultation is legally exempt.
Patients are responsible for ensuring that receiving a teleconsultation complies with their national and local regulations.

Services will not be provided to persons located in countries subject to U.S. sanctions.

2. No Emergency Services

Premier Neuro Health does not provide emergency or crisis care.
If you experience a medical emergency, call 911 or visit your nearest emergency department.

3. No Medical Relationship via Website

Accessing or reading website content does not create a physician–patient relationship. A formal relationship is established only upon registration, consent, and completion of an initial consultation.

4. Intellectual Property

All content, including text, images, and logos, is the property of Premier Neuro Health. Unauthorized reproduction or distribution is prohibited.

5. Limitation of Liability

Services are provided “as is.” Premier Neuro Health is not liable for indirect, incidental, or consequential damages arising from website use or telehealth platform access.

6. Indemnification

You agree to indemnify and hold harmless Premier Neuro Health, its affiliates, and Dr. Souayah from any claims or damages arising from your misuse of the site or violation of these Terms.

7. Telehealth Services

By engaging in telehealth visits, you consent to the use of secure video technology via Athenahealth, acknowledge the potential for technical limitations, and accept that no guarantees are made regarding outcomes or connectivity.

8. Arbitration

Any dispute arising from these Terms shall be resolved through binding arbitration in the State of New Jersey, in accordance with the rules of the American Arbitration Association.

You waive the right to trial by jury or to participate in class actions.

9. Governing Law

These Terms are governed by the laws of the State of New Jersey, without regard to conflict-of-law principles.
